This patient reported to me with the complaint of facial swelling for past 5 months which became noticeable for last 10 days. After thorough clinical, radiological and histopathological examination, Maxillofacial Surgeon Dr. S. M. Surgical treatment was planned to remove the entire cystic lesion and reconstruct the affected jaw portion in a single stage. Costal rib graft was harvested for the reconstruction of the mandible. Through intraoral approach, crevicular incision was placed in the mandibular anterior teeth region extending posterior till mandibular notch, gingivomucoperiosteal flap was raised, cystic lesion was surgically exposed and the affected portion was removed along with the involved teeth in toto and reconstruction was done using long titanium bone plate and costal graft maintaining the contour of the mandible. After successful graft uptake, dental rehabilitation will be done using dental implants in the newly formed bone.

Soft Jaw
Hemifacial Microsomia is a birth deformity affecting the bony skeleton and soft tissues of the craniofacial region. It is typically characterized by pronounced asymmetry of the face due to deficient growth of the lower jaw (mandible) on one side, deficient soft tissues of the face and underdeveloped ear (microtia). Bony and soft tissue structures on one side of the face grow lesser than the other side. The affected side of the face appears disproportionately smaller than the other. Commonly affected are the lower jaw, eye, ear, facial nerve and cheek. The most significant of these deformities is that of the jaws, which when corrected, minimizes the entire deformity and makes the face look more symmetric.

Socket Eye
Eye is one of the key senses that is considered very vital and held close to heart. In case of injuries to the eyes, careful and critical evaluation is performed by a maxillofacial surgeon in addition to a specialist (Ophthalmologist). Initial inspection should not involve manipulation or touching the injured area till fractures of bones around the eye are ruled out. An expert will ask to move the patient’s eye in all direction to ensure full extent of function, look for the shape, color and vision of the eyeball. Special tests may be performed as needed. The fractures of the bones of the eye need to be assessed by imaging techniques. Fractures of orbital (eye socket) bones could be of different types depending on the nature of impact, extent of injury, age of the patient besides numerous other factors. These orbital fractures could be anywhere in the adjacent area including cheekbone (zygoma), above the eye, below the eye, or in the bones surrounding the eye socket. Early diagnosis of these fractures is important to avoid some later complications and sequel. Laceration around eyes needs to be carefully worked on as contraction of scars may itself cause loss of vision.

Dental Root Elevators
Our prosthodontic and endodontic team expertly planned to save that tooth and also give her a beautiful smile. Endodontic specialist performed root canal treatment for that tooth. Severely curved apical third of root was completely sealed with flowable sealants and lateral canals were also filled completely. Using lateral condensation technique the rest of the root canal was filled. Results are shown in the post operative radiographs. Later the tooth will be covered with ceramic crown. The patient was very happy as her natural tooth was saved and restored successfully. Post operative prosthodontic results will be uploaded after fixing the crown

Craniofacial surgeon Dr. S.M. Balaji skillfully performed the complex reconstruction. A cortical bone graft was harvested from the rib and used to reconstruct the left forehead. Miracle protein rhBMP-2 was placed on the bone graft to stimulate new bone formation. The defect in the nasolacrimal duct was corrected with DCR or Dacryocystorhinostomy. The ostium was opened with rongeurs, bone adjacent to nasolacrimal sac was removed. A new direct communication was created between the nasolacrimal sac & the nasal mucosa by placing a silicone tube to enable the tears to drain into the nose. Complete craniofacial defect was corrected successfully.

Bone Curettes
Maxillofacial Surgeon Dr. S. M. Balaji adroitly planned to remove the infected bone graft and plate and to reconstruct the lower jaw in a single surgery. He approached the surgical site extraorally through the previous scar region and removed the dislodged bone plate and sequestrum completely. Adjacent infected bone was removed with bone curette and a new NiTi bone plate was fixed to the adjacent healthy bone. To enhance the rigidity, costal graft was harvested and placed near to the bone plate with rhBMP-2 to increase the efficiency of new bone formation. Mucosal flaps were approximated in layers with very minimal scar formation.
