ACNE.. is an inflammatory disease of the sebaceous glands and hair follicles of the skin characterized by comedones, papules and pustules; usually associated with seborrhea. Cysts and nodules may develop and scarring is common. Acne is marked by either papules about comedones with black centers (pustules) or hypertrophied nodules caused by overgrowth of connective tissue. In the indurative type the lessons are deep-seated and cause scarring. The face, neck, and shoulders are common sites. Acne may be obstinate and recurrent.Etiology : The cause is unknown, but predisposing factos include hereditary tendencies and disturbances in the androgen-estrogen balance. Acne usually begin at puberty, when the increased secretion of androgen in both males and females increases the size and activity of the pilo-sebaceous glands. Specific inciting factors may include food allergies, endocrine disorders, therapy with adrenal corticosteroid hormones, and psychogenic factors. Vitamin deficiencies, ingestion of halogens and contact with chemicals such as tar and chlorinated hydrocarbons may be specific causative factors. The fact that bacteria are important once the disease is present is indicated by the successful results following antibiotic therapy. The lesions may become worse in women and girls before the menstrual period.Treatment : Before treatment, patients should consult a dermatologist to ensure that the skin condition is acne. Rashes resembling acne can be caused by cosmetics or other skin irritants. Treatment may be local or systemic. Topical cleansing and peeling (keratolytic) agents and tetracycline may be used during the last 2 trimesters of pregnancy, and not prior to age 12 to prevent tooth discoloration. Topical application of Vitamin A acid is effective. Severe cystic acne responds well to orally administered isotretinoin, but because this drug is teratogenic (cancer producing), it can not be used in women of child bearing age unless they are not sexually active or effective contraceptive use is guaranteed. Comedones should be extracted carefully and cysts and pustules incised and drained. Conditions known to cause exacerbations should be avoided.Pimple : Pimple is papule or pustule of the skin, sometimes going on to suppuration; often seen in clusters on the skin of the adolescent with acne. Patients should be warned not to pick at pimples because infection may result.Dandruff : Dandruff is the normal ex-foliation of the epidermis of the scalp in the form of dry white scales. Several over-the-counter products including salicylic acid, pyrithione zinc, sulfur, selenium sulfide and coal tar are approved for treating the condition but each one has its own side effects.Scabies : Scabies is a highly communicable disease caused by an arachind, Sarcoptes scabiei, variety hominis the itch mite. The impregnated females live in burrows that appear as slightly discolored line several centimeters in length. The eggs are deposited within the tunnel hatch within 3 to 5 days. The larvae molt into nymphs on the skin surface and mature in 2 to 3 weeks. Mature insects mate, and the gravid female invades the skin to begin the cycle. Scabies is transmitted by direct contact with infected persons, and rarely by fomites.Symptoms : Scabies manifests as papules, vesicles, pustules and burrows and causes intense itching resulting in eczema. The body parts most commonly affected are the hands, between the fingers, the wrists, axillae, genitalia, beneath the breasts and the inner aspect o the thighs.Diagnosis : Mites are identified in the tissue scraped from burrows, lesions or fresh papules.
